Job Description:

You will play a critical role in managing symptoms, administering medications, educating patients and their families, and ensuring all goals of care are being met to the highest of standards.
Key Responsibilities:
*Conduct initial and ongoing assessments of the patient's physical, emotional, and spiritual needs.
*develop and implement individualized care plans in collaboration with the primary hospice care team
*Administer medications and treatments as prescribed by the attending physician.
*Monitor patients conditions and report any changes to the hospice care team and attending physicians to advocate for the patients.
*Ensure documentation is completed in an accurate and timely manner.
*Participate in team meetings to review patient care and update care plans as needed
*Maintain confidentiality and adhere to professional standards and ethics
Qualifications:
*Current and unrestricted nursing license in the state of North Carolina
*Minimum of 2 years nursing experience with at least 1 year of hospice or palliative care preferred.
*Strong clinical skills with the ability to manage complex patient care needs.
*excellent communication and interpersonal skills, with a compassionate and empathetic approach to patient care.

We are seeking a compassionate and skilled Triage/Call-Center Hospice nurse to join our team. As a Triage hospice RN, you will be responsible for providing telephone support to patients, families, and caregivers. You will assess and manage patient symptoms, provide education, and coordinate care with the hospice team, ensuring that our patients receive the highest level of care at all times..
Key Responsibilities:
*provide telephone triage and support to hospice patients, families, and caregivers
*Conduct initial and ongoing assessments of patient's needs and symptoms over the phone
*Develop and implement care plans in collaboration with the hospice team
*administer symptom management and provide education on medication and care practices
*Coordinate with on-call nurses and other hospice team members for in-person visits as necessary.
