What is Hospice?
Hospice is a type of care focused on providing comfort and support to individuals with terminal illnesses, typically when curative treatment is no longer an option or desired. The primary goal of hospice care is to enhance the quality of life for patients and their families by managing pain and symptoms, addressing emotional and spiritual needs, and offering support through the end-of-life journey. Our goal at Coulter’s hands is to help people live as fully and comfortably as possible for the time they have left, surrounded by the people they love most.
With Coulter’s Hands, we offer an in-home service unlike any other. By adopting the philosophy of co-created care, we empower our clients and families to actively participate in the care journey. We believe this model extends beyond nursing and is about building relationships, encouraging open communication, and providing education without over-complicating things. This philosophy guarantees that every care team member has a voice that is not just heard but truly valued.
What is End-Of-Life?
End-of-life refers to the final phase in a person’s life when they’re approaching death. This period is characterized by a range of physical, emotional, and spiritual changes and is often associated with terminal illness or advanced irreversible conditions. End-of-life care focuses on supporting the individual and their loved ones and providing comfort in all life’s final moments.

Coulter’s Hands staff are experienced in monitoring signs/symptoms of a terminal disease progression and general decline.
Common signs/symptoms of disease progression include: pain, nausea/vomiting, shortness of breath, agitation, increased weakness, difficulty swallowing, increase in sleep, and decrease in appetite.

Development of individualized care plans tailored to the specific needs of the patient.
Patient care plans are created in collaboration with the patient, family, and the primary hospice team..

Coulter’s Hands staff will educate and support patients and family members on disease progression, symptom management provided by the primary hospice team, and caregiving techniques.
While caring for their loved ones, providing support and education to the family, will ensure they have the confidence they need in the absence of the primary hospice team and/or Coulter’s Hands nurses and nursing assistants.

Medication administration by an experienced registered nurse in hospice care.
Coulter’s Hands RN will monitor for effectiveness of medications and notify the primary hospice provider to report any uncontrolled symptoms in pain/agitation/respiratory distress.
Comfort medications are provided by the primary hospice team.

Coulter’s Hands staff will provide emotional support to patients and their families by showing love, compassion and prayer.

Offering complimentary therapies such as massage therapy, music therapy, art therapy, pet therapy, movie night, in-home hair appt, and aromatherapy to enhance patient comfort and well-being. (add-on service)

Round-the-clock availability of hospice trained and experienced registered nurses and nursing assistance to bridge the gap of the primary hospice team visits.

Coordination of care with the patient’s primary hospice provider to ensure all patient needs are met.
The primary hospice provider will be notified by our staff immediately to report a change in condition, uncontrolled pain/symptoms, falls, and any medication refills.

Coulter’s Hands staff will provide assistance with ADLs such as bathing, dressing, grooming, and mobility ensuring patient comfort and dignity.

Offering temporary relief for family caregivers by providing professional care for the patient, allowing caregivers to rest and recharge.
