Welcome to Coulter’s Hands In-Home Concierge Nursing Services. These Terms and Conditions govern your use of our services and website. By accessing or using our services, you agree to be bound by these Terms. If you do not agree with any part of these Terms, please do not use our services.

1. Services Provided

Coulter’s hands registered nurses and nursing assistants provides personalized nursing and caregiving services in the comfort of your home. Our services include, but are not limited to, medication management/administration, assisting with activities of daily living (bathing, dressing, and grooming). Meal preparation and feeding assistance, light house keeping and laundry, errand running and shopping assistance, wound care, ongoing emotional support and spiritual support for both patient and family, monitoring End-of-Life signs/symptoms, facilitate communication/coordination of care between patient/family and the primary hospice team, End-of-Life education without overcomplication, sensitive and dedicated patient advocacy, respite care, and travel nursing care.

2. Eligibility

To use our services, you must be at least 18 years of age and have the legal capacity to enter into a binding agreement. By using our services, you represent and warrant that you meet these eligibility requirements. Patient must be under the Medicare hospice benefit and have a primary hospice provider that oversees patient with an interdisciplinary team.

3. Client Responsibilities

By using our services, you agree to:

- Provide accurate and complete information about your health and care needs.

- Follow the care plan and instructions provided by our nursing staff.

- Ensure a safe environment for our staff to deliver care, including but not limited to, keeping pets secure and providing a smoke-free environment.

- Notify Coulter’s Hands, and the primary hospice team of any changes in your health condition, care needs, or other relevant circumstances.

4. Payment and Billing

a. Fees:

You agree to pay all fees for the services provided as outlined in your service agreement. Our fees are subject to change, and you will be notified of any changes in advance.

b. Payment Terms:

Payment for services is due at the time of service, unless otherwise specified in the service agreement. Acceptable forms of payment include: cash, check, or credit card. The client/primary caregiver (family) signature on consent form during admission, authorizes the use of credit care information on file for all appointments provided by Coulter’s Hands.

c. Late Payments:

If you fail to make a payment when due, we may charge a late fee or suspend services until your account is brought current. A $25 fee will be applied for any check returned unpaid by the client’s bank. By signing the service agreement consent form during the time of admission, client agrees to pay $45/hr for nursing assistant services, and $95/hr for registered nursing services. Hourly rates are subject to change and clients will be notified by Coulter’s Hands promptly. Failure to pay for services provided by Coulter’s Hands may result in the discontinuation of service. All outstanding balances are due immediately upon termination of services.

5. Cancellation and Rescheduling

a. Cancellation Policy:

You may cancel or reschedule services by providing at least 24 hours' notice. Cancellations with less than 24 hours' notice may be subject to a cancellation fee of $50, unless client has passed, has been admitted to the hospital/hospice house, or any unforeseen circumstances.

b. Service Termination:

Either party reserves the right to terminate or suspend services with written notice. If these Terms are violated or if we determine that the service environment is unsafe for our staff, services will terminate immediately.

6. Limitation of Liability

To the maximum extent permitted by law, Coulter’s Hands shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages, including, without limitation, damages for loss of profits, goodwill, use, data, or other intangible losses, resulting from:

- The use or the inability to use our services.

- Any unauthorized access to or alteration of your transmissions or data.

- Statements or conduct of any third party on our services.

- Any other matter relating to our services.

7. Indemnification

You agree to indemnify, defend, and hold harmless Coulter’s Hands, its affiliates, officers, directors, employees, and agents from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or related to your use of our services, your violation of these Terms, or your violation of any rights of another party.

8. Privacy and Data Protection
