During the initial consultation and in-home visit, our Client Care Coordinator meticulously documents your home care requirements and personal preferences to ensure an impeccable caregiver match. The Coordinator then selects the most exceptional caregiver and will schedule an introduction between you, the caregiver, and the Coordinator. This introductory meeting ensures that all parties review and fully understand the Plan of Care, guaranteeing a seamless and harmonious care experience.

At Coulter’s Hands, we pride ourselves on maintaining seamless, uninterrupted care through our elite team of highly qualified caregivers. Should your primary nurse or CNA be unavailable due to illness or vacation, we will promptly arrange for an interim caregiver and ensure you are informed of the change in advance. Our Client Care Coordinator will personally introduce the substitute caregiver and meticulously review your Plan of Care with them to guarantee the highest level of service continuity.

Palliative care and hospice care both focus on providing relief from symptoms and improving quality of life, but they differ in their scope, timing, and goals:
Palliative Care
Purpose: Aims to relieve symptoms, pain, and stress associated with serious illnesses, regardless of the stage of the disease. It focuses on improving the overall quality of life for patients and their families.
Timing: Can be provided at any stage of a serious illness, from diagnosis through treatment, and can be given alongside curative or life-prolonging treatments.
Scope: Offers a holistic approach, addressing physical, emotional, and spiritual needs. It can be provided in various settings, including hospitals, outpatient clinics, and at home.
Eligibility: Available to patients of any age or stage of illness, not limited to those with terminal conditions.
Hospice Care
Purpose: Provides comfort care and support for individuals who are in the final stages of a terminal illness, with a focus on ensuring a peaceful and dignified end-of-life experience.
Timing: Typically provided when a patient is expected to have six months or less to live and has decided to forego curative treatments in favor of comfort care.
Scope: Emphasizes comfort and quality of life rather than cure, offering services to manage symptoms and provide emotional, spiritual, and practical support. It is generally provided in the patient’s home, hospice facilities, or sometimes in nursing homes.
Eligibility: Specifically designed for patients with terminal diagnoses, and services continue as long as the patient meets the criteria and the focus remains on comfort.
While both types of care aim to improve quality of life, palliative care can be used alongside curative treatments and at any stage of illness, while hospice care is specifically for patients nearing the end of life who are no longer pursuing curative treatments.

At this time, we are private pay only. However, we will work with our clients to assist in reimbursement from both primary health and long-term care insurance. Please contact your individual insurance carrier to confirm eligibility.

The cost of hiring a private nurse for hospice care will vary depending on the level of assistance you need and the total hours of care. Hospice can last anywhere from a few days to months. It will also depend on the nurse’s licensure, experience, and training.
